An orthopedic hand specialist is really the only person who would be able to accurately form a diagnosis and plan for your hand. I know that TFCC tears frequently require surgery and always require immobilization (sounds like you were already immobilized). It sounds like you've got a lot going on in there though and there could be significant structural damage. There's probably not a lot your PT could do for you.
